プラスミドの菌種間伝播を伴うニューデリー・メタロ-β-ラクタマーゼ 1（NDM-1）によるサルモネラ（Salmonella enterica）の病院アウトブレイク★

ニューデリー・メタロ-β-ラクタマーゼ（NDM）遺伝子は、カルバペネム系薬を含む一連のβラクタム系薬に対する高レベルの耐性を付与する。ショートリードおよびロングリードシークエンシングを用いて、NDM 陽性腸内細菌目細菌によるアウトブレイクについて、サルモネラ（Salmonella enterica）と肺炎桿菌（Klebsiella pneumoniae）の間でのNDM 陽性プラスミドの遺伝子水平伝播イベントの可能性も含めて検討した。ゲノム解析から、患者 1 から分離された肺炎桿菌のNDM 陽性プラスミドと、患者 2 から分離されたS. enterica、肺炎桿菌、およびクレブシエラ・オキシトカ（Klebsiella oxytoca）の陽性プラスミドとの間に高度の類似性が示され、これにより菌種間伝播イベントが確認された。従来の感染制御サーベイランスではこれまで検出されなかった院内アウトブレイクにおける、全ゲノムシークエンシングの有用性が実証された。
監訳者コメント：
複数の菌種のメタロ-β-ラクタマーゼ陽性腸内細菌目が検出された場合はプラスミド性にメタロ-β-ラクタマーゼ遺伝子が菌種を越えて伝播したのか、それとも別個のイベントが起きているのか判断が必要になる。本研究では菌種を越えた伝播を確認しており、全ゲノムシークエンシングの有用性を示している。
