コロナウイルスに対する紫外線発光技術の有効性:システマティックレビュー★

現在進行中である新型コロナウイルス感染症(COVID-19)のパンデミックにより、病院および市中環境において効果的な感染予防・制御(IPC)策を採用する重要性が明確に示されている。紫外線(UV)ベースの技術は有望な IPC のツールであり、衛生管理への効果的な適用はこれまでに広く評価されてきたが、新型コロナウイルス(SARS-CoV-2)の伝播に対する効果に関して入手できるエビデンスは不十分、不均一であり不確定である。コロナウイルスに対する UV 技術の有効性に関して入手できるエビデンスを集積することを目的に、Medline、Embase、Cochrane Library、ならびに主な臨床試験のレジストリ(WHO ICTRP、ClinicalTrials.gov、Cochrane および EU Clinical Trial Register)の検索を行い、PRISMA ガイドラインに従ってシステマティックレビューを実施した。試験介入に関する定量的データを表にまとめ、異なるコロナウイルスの種と系統、UV 供給源、UV 曝露の特性および結果に基づいて集積した。1972 年 から 2020 年に発表された 18 報が我々の組入れ基準を満たした。6 報は SARS-CoV-2 に、4 報は重症急性呼吸器症候群コロナウイルス‐1(SARS-CoV-1)に、1 報は中東呼吸器症候群コロナウイルス(MERS-CoV)に、3 報は季節性コロナウイルスに、4 報は動物コロナウイルスに焦点を合わせたものであった。すべて実験研究であった。概して対象の研究は全体に不均一であったにもかかわらず、表面上のまたはエアロゾル化したコロナウイルスを SARS-CoV-2 を含め完全に不活化するには、最大 15 分の曝露時間がかかり UV 光源からの最大距離は1 m 以下を要することが報告された。衛生管理の分野における UV ベースの技術の進歩と SARS-CoV-2 に対する証明済みの強い殺ウイルス作用の潜在力は、病院および市中環境での IPC を目的とする使用と COVID-19 パンデミック終了への寄与を支持するものである。医療現場と非医療現場どちらでも効果的な IPC を目的とした UV 技術適用の有効性と安全性の両方を確保するため、国内および国際的なガイドラインを改訂すべきであり、使用のパラメータおよび条件を明らかにする必要がある。
監訳者コメント:
コロナウイルスに対する紫外線の有効性は明らかではあるものの、現場で利用する装置とその性能が実際の有用性に大きく影響すると思われた。
