適切な分子生物学に基づく方法の適用は、B 群レンサ球菌の検出結果の感度を有意に高める★

背景
Streptococcus agalactiae(B 群レンサ球菌)は、新生児の早期発症・晩期発症疾患の主な原因である。妊娠女性における B 群レンサ球菌保菌検査の確実な結果は、新生児感染症の発生率および死亡率を低減させる可能性がある。
目的
従来の培養検査と米国食品医薬品局(FDA)承認の核酸増幅検査(BD Max GBS[Becton Dickinson 社])の結果を比較すること、ならびに標準的ポリメラーゼ連鎖反応(PCR)の独自のプロトコールを確立すること。
方法
B 群レンサ球菌保菌を評価するために、ポーランドのビドゴシュチの病院 3 施設で採取された膣および直腸スワブ計 250 検体を用いた。標準的な検査法(増菌培養培地で一晩培養)の結果を、BD Max GBS アッセイ(Becton Dickinson 社)および 2 つの標準的 PCR プロトコールの結果と比較した。この PCR プロトコールは、増菌液体培地で一晩培養した検体から S. agalactiae の cfb 遺伝子および 16S rRNA 遺伝子を検出するために確立された。
結果
全体的な B 群レンサ球菌保菌は16.4%~ 23.2%と推定され、適用した検出法に左右された。各試験で得られた陽性結果率がもっとも高かったのは、BD Max GBS アッセイの適用による。これらの特定の方法により得られた陽性結果数の差は統計学的に有意であった。全体的に見て、比較した方法の適用により観察された結果には 27 の不一致がみられた。
結論
B 群レンサ球菌検出のために適用した方法は感度が異なる。培養法は非常に特異的であるが、市販の BD Max GBS アッセイ、または今回の目的のために確立された院内 PCR プロトコールと比較して、S. agalactiae 検出の感度は低いようである。
監訳者コメント:
培養法による菌検出には一定数の菌量が必要となり、特異度が高くても感度が下がる可能性がある。GBS のスクリーニングは我が国でも実施されており、簡易かつ迅速で低価格な検査方法が登場することは望ましい。
