Routine antiseptic baths and MRSA decolonization: diverse approaches across Singapore’s acute-care hospitals

シンガポールの急性期病院 17 施設におけるメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）に対するサーベイランス、ならびに MRSA 保菌患者の管理に関する実践の多様性を明らかにするために、シンガポール保健省は 感染予防と管理の指導者らと共同で委員会を開催した。すべての病院が、国内方針と一致して、入院時の普遍的 MRSA スワブ検査をほぼ実践していた。しかし、施設によってサーベイランス・スワブ検査の陽性結果および陰性結果の両方に対する対応には、ルーチンの消毒浴の役割および MRSA 除菌などを含め、大きなばらつきが認められた。除菌を実行している施設のほとんどが、除菌の役割を長期のクリアランスではなく「バイオバーデン減少」であると考えていた。

監訳者コメント：

シンガポールでは積極的に消毒浴や MRSA の除菌が行われているようである。日本では単なる除菌を目的として抗 MRSA 薬を使用することを控えるべきとしていて、術後感染のリスクが高い場合にムピロシン軟膏の鼻腔塗布やクロルヘキシジンの消毒浴が推奨されている。
