Investigation and control of an outbreak due to a contaminated hospital water system, identified following a rare case of Cupriavidus pauculus bacteraemia

背景

Cupriavidus pauculusは、臨床感染の原因としてまれである。本稿では、給排水設備の汚染に続発した小児血液腫瘍病棟における C. pauculus や他のグラム陰性菌による菌血症のアウトブレイクについて報告する。

目的

新設の病院における水系感染症のアウトブレイクに対して実施された調査および制御策について報告すること。

方法

水道設備内のさまざまな箇所での広範な水質検査を行った。蛇口、シャワーヘッド、整流器を含む部品の微生物学的分析を実施した。排水管からスワブ検体も採取した。小児血液腫瘍病棟におけるグラム陰性菌感染症のサーベイランスを確立した。

結果

水質検査により、給排水設備の広範な汚染が明らかになった。整流器を含む蛇口もひどく汚染されていた。排水管には根本的な構造上の異常があることが確認された。水質検査により、膨張管や蛇口など水道設備内のリスクの高い要素の検出が可能になり、その結果は仮説生成の一助となった。稼働前データの再考およびリスク評価から、開院前後の水道設備内に存在する広範なリスクが示された。

結論

水系微生物による感染症を予防するために、病院の水道設備の慎重な設計、適切な制御策、維持管理が不可欠である。われわれは、今後の予防を視野に入れて、今回の事象から学べるポイントについて検討した。

監訳者コメント：

2015年春に新規開設された英国の病院で2018年に水道設備の汚染による菌血症が発生した。2016年には無菌調整薬局の水道からCupriavidus pauculusが検出されていたが、今回の事例に関する調査では検出されていなかった。水道系統からは複数の細菌が検出されており、消毒に使用されたのは、銀イオンと過酸化水素の製剤であったが、効果が不十分であった、としている。日本においても同様に、レジオネラ菌やグラム陰性菌がひとたび水道管から検出されるとその除去は相当に困難であることを再認識した。
