背景
COVID-19 伝播の低減は、飛沫およびエアロゾルの拡散を制御することにかかっている。フルオレセイン染色により顕微鏡的飛沫の存在が明らかになる。
目的
非層流換気システムおよび層流換気システムの手術室における飛沫拡散を比較すること。
方法
「咳発生装置」を手術室トロリーに45度の角度で設置した。フルオレセインで染色した「分泌物」を、一連の測定用ターゲットに噴霧した。これらのターゲットを紫外線照射下で撮影し、飛び散った飛沫の大きさと距離を「汚染源検出」ソフトウェアで評価した。
結果
最も小さい飛沫は約 120μm、最も大きい飛沫は約 24,000μm であった。検出された平均スポット数は、非層流換気システムの手術室では 25,862 カ所であったのに対し、層流換気システムの手術室では 11,430 カ所であった(56%の減少)。層流換気は、主に小さい飛沫(1,000μm 未満)に影響していた。飛沫で覆われた表面積の割合は、非層流換気システムの場合は 50 cm の距離では 6%、2 m の距離では 1%、および 3 m の距離では 0.5%であり、層流換気システムの場合はそれぞれ 3%、0.5%、および 0.2%であった。
結論
臨床環境における飛沫拡散の正確なマッピングは、フルオレセイン染色と画像解析を用いれば可能である。層流換気は小さい飛沫に影響を及ぼしたが、筆者らの「エアロゾル生成手技」に基づく咳モデルでは、大きい飛沫への影響は限られていた。本研究の結果は、層流換気システムの手術室では非層流システムの場合と同様の術後清掃が必要であること、また手術室スタッフは中リスクおよび高リスクの患者に対応する時は個人防御具を完全に装備することを考えるべきである。
監訳者コメント:
手術室の飛沫感染リスクを明らかにする研究。層流換気システム、非層流にシステムにかかわらず、3 mは飛沫が飛んでおり、術後清掃と個人防護具の適切な着用が重要である。
