Where to do water testing for Pseudomonas aeruginosa in a healthcare setting

緑濃菌（Pseudomonas aeruginosa）は、院内の給水設備に広く存在している重大な院内感染の原因微生物である。英国保健省の Health Technical Memorandum 04-01 は、外来患者の透析室など重点的にケアを行う箇所で水質検査を推奨することによってもたらされるリスクについて述べている。本稿では、教育病院 2 施設がいかに外来血液透析患者のリスクについて個々に精査し、同じ結論を導き出したかについて考察する。緑膿菌の感染エピソードが最も多く認められたのは救命治療科であり、次いで熱傷科、血液内科が多く、血液透析科が最も少なかった。これらの結果に基づき、救命治療科、熱傷科、血液内科などの領域では水質検査を実施すべきであり、外来血液透析では実施すべきでないことを提案する。

監訳者コメント：
緑膿菌感染のリスクが医療機関内のどの部門・箇所で相対的に高く、かつ環境因子が大きく関与しているかどうか評価することは、個々の医療機関における本菌の感染制御上、非常に重要である。この意味において、給水中の緑膿菌検査実施の要否も左右されるであろうが、検出された場合の解釈と対応については、難しい点をはらむ。部門それぞれの状況とリスクを、詳細かつ総合的に勘案した対応が、求められるといえる。
