「二次的バイオフィルム」は過酢酸による内視鏡の高水準消毒の失敗を引き起こす可能性がある

緒言
消毒薬に対するバイオフィルムの感受性の低下は、医療器具の再処理の成功に対し課題を突きつけている。本研究では過酢酸(PAA)による事前の消毒後も残存しているバイオマスが、その後の成熟した緑膿菌(Pseudomonas aeruginosa)バイオフィルムの PAA への耐性に対して与える影響を検討した。緑膿菌バイオフィルムの細胞外高分子物質の特定成分の酵素分解がPAA の消毒効果に対して与える影響も評価した。
方法
平板菌数計数法を用いて生存能力を評価することにより、PAA によって死滅したバイオフィルムのバイオマス上に増殖したバイオフィルムの PAA に対する感受性を、24 ウェルプレートのウェルで増殖したバイオフィルムの PAA 感受性と比較した。PAA がバイオフィルムのバイオマスに対して及ぼす効果は、バイオフィルムのバイオマス総量のクリスタルバイオレット法による定量を用いて測定し、PAA がバイオフィルムの細胞外高分子物質の多糖体およびタンパク質成分に対して及ぼす効果は、それぞれフェノール硫酸法またはブラッドフォード法を用いて定量した。残存しているバイオフィルムのバイオマス上に増殖したバイオフィルムにおける生細胞と死細胞の分布を視覚化するため、共焦点顕微鏡を使用した。
結果
事前に消毒されたバイオフィルム由来の残存バイオマスの存在は、その後のバイオフィルムの耐性を著しく高めた。消毒されたバイオマス上に形成され 96 時間経過した「二次的バイオフィルム」は 4,000 ppm の濃度の PAA で生存したが、この濃度は実臨床で高水準消毒に使用される濃度を上回る。
結論
これらの知見は、特定の条件下では残存している細胞外高分子物質の再定着が内視鏡のような医療器具の消毒失敗を引き起こす可能性があることを示唆しており、内視鏡の消毒前の洗浄の重要性を強調している。
