病院には何名の感染制御担当職員が必要か?★★

1日のワークショップにおいて、モデル病院で感染制御活動を遂行するのにどれくらいの時間が必要かについて、熟練した感染制御専門家と医学微生物学者が討論した。彼らは、感染制御専門家は178床に対して専従換算※1名、医学微生物学者は806床に対して専従換算1名が基準であることに同意した。これは、通常の基準※※よりそれぞれ40%、24%多い。公表病床数は、病院業務の指標として不適切となりつつあるため、新たな基準として入院数が提案されている。この基準では、感染制御専門家は入院5,000例につき専従換算1名、医学微生物学者あるいは疫学者は入院25,000例につき専従換算1名である。

監訳者コメント:
オランダで16名のICPと10名の医学微生物学者(日本の病院では微生物検査技師が行っている業務を担当する人々)を集めて、意見聴取した結果をまとめた論文。ICPが178床に1名という数字は、有名なSENICスタディの結果である250床に1名という結果より、必要なICPを多く見積もっている。この背景には、SENICスタディが行われた1970年代と現代の医療環境が著しく異なっていることが挙げられる。また、今回の数字は、現代のICPに求められる業務に必要な時間を積み上げて得られた結果である。それらを考えると、今回の結果は妥当であり、いまだに標準とされているSENICスタディの結果を使用した感染対策もそろそろ見直されてもよいのかもしれない。
