Disinfection of artificially contaminated gloved hands reduces transmission of Staphylococcus epidermidis to catheter valves

肉眼的に汚染されていない手袋が、同一患者への複数の臨床行為の際に使用される状況において、手袋着用の手指の消毒がますます推奨されている。このような消毒の実践が、臨床行為の際の細菌伝播の低減に寄与することを示すデータはないので、標準化された実験的研究を実施した。手袋着用の手指を消毒済みまたは未消毒の条件で、表皮ブドウ球菌（Staphylococcus epidermidis）で汚染された鶏の胸肉に接触させた後、滅菌カテーテルバルブに接触させた。汚染された手袋により、カテーテルバルブに対し 5.18 log₁₀ コロニー形成単位（cfu）の表皮ブドウ球菌が伝播した。汚染された手袋の消毒によって、伝播数は 0.78 log₁₀ cfu と有意に減少した。手袋着用の手指の消毒は伝播リスクを低減させる可能性がある。

監訳者コメント：
手袋の上から手指衛生を行うことは、手袋の素材との相性もあり必ずしも推奨されていない。手袋を装着し続けることは医療従事者自身の曝露を低減する効果があっても、患者対応としては手袋を必要に応じて頻回に交換し、この間に手指衛生を行うことが基本となる。
