Spread of ESBL-producing Escherichia coli in nursing home residents in Ireland and the Netherlands may reflect infrastructural differences

ナーシングホーム 2 施設（オランダ、アイルランド各 1 施設）における有病率研究により、オランダの入居者 4 例（11％）およびアイルランドの入居者 6 例（9％）が 11 の基質特異性拡張型β-ラクタマーゼ（ESBL）産生大腸菌（Escherichia coli）を保菌し、そのうち 10 は CTX-M-15 を含むことが認められた。同じ病棟の入居者 3 例に由来するオランダの分離株 4 つは大腸菌 O25：H4、配列型（ST）131 に属しており、全ゲノムシークエンシングによると同じクラスター型の一部であった。異なる 3 病棟のアイルランドの入居者 4 例は、同一の大腸菌 O89：H9、ST131、複合型1478 を保菌していた。アイルランドの 3 病棟の間の交差伝播は、ナーシングホームのインフラの違い、特に共用部分およびベッドが複数ある部屋に関する違いを反映している可能性がある。

監訳者コメント：
ナーシングホームにおける ESBL 産生大腸菌の保有状況をオランダとアイルランドで比較した研究である。アイルランドでは複数ベッドの部屋が多く、共用部分も限られ多くの居住者が利用するため、病棟をまたいだ交差感染が発生していたことから、ナーシングホームのインフラ（設備）が微生物の伝播に関与すると指摘している。納得の結果である。
