Timing, diagnosis, and treatment of surgical site infections after colonic surgery: prospective surveillance of 1263 patients

背景：
手術部位感染症（SSI）は結腸直腸手術後にもっとも頻発する合併症であり、入院期間と費用に大きな影響を及ぼす。
目的：
結腸手術後 30 日以内の SSI の発生率、タイミング、治療について分析すること。
方法：
本研究は、Lausanne University Hospital（CHUV）における 2012 年 2 月から 2017 年 10 月までの連続した結腸手術症例の後向き分析による質改善プロジェクトである。独立した全国的サーベイランスプログラム（www.swissnoso.ch）により、SSI を術後 30 日まで前向きに評価した。感染症に対するドレナージ（直接的創部開口部、経皮的）および外科処置を含む治療法を評価した。
結果：
研究コホートは患者 1,263 例で、緊急対応として 532 例の処置を実施した（42％）。SSI が患者 271 例（21％）にみられ、中央値で術後 9 日目（四分位範囲［IQR］4 ～ 16）に発生した。具体的には、53 例（4％）は切開部表層、65 例（5％）は切開部深層、153 例（12％）は臓器／腔（吻合不全も含む）の感染症であった。切開部表層 SSI は中央値で術後 10.5 日目（IQR 7 ～ 15）、切開部深層 SSI は中央値で術後 10 日目（IQR 8 ～ 15）、臓器／腔 SSI は中央値で術後 8 日目（IQR 5 ～ 11）に発生した。64 例（24％）は退院後に診断された。術後 7 日目までに検出されたのは、臓器／腔感染症の 47％であったのに対し、切開部表層および切開部深層の感染症では 26％のみであった（P ＝ 0.003）。133 例（49％）が外科的処置を必要とし、それ以外の症例は、全身麻酔を施行しないドレナージにより治療された（138 例［51％］）。
結論：
臓器／腔感染症は術後早期に発生したのに対し、切開部感染症の大部分は 30 日間の全観察期間を経過した退院後に検出された。このことは、組織的で、徹底かつ独立したサーベイランスプログラムによる適切な追跡調査の重要性を強調するものである。
