大部分のガイドラインは、内視鏡用送水ボトルを少なくとも毎日、新たに殺菌したまたは高レベル消毒を行った送水ボトルと交換することを推奨している。しかし、これらの推奨はすべて、主として専門家の意見やアウトブレイクの報告に基づいたものである。著者らは、送水ボトルを最長 5 日間使用する消化管内視鏡検査部門で用いられる送水ボトルの水の質を検討した。その結果から、送水ボトルの再使用を 1 日を超えて行うことは不適切であることが示された。送水ボトルを毎日交換することを勧める専門家の意見は、5 日後に交換するよりも理にかなっており、より安全な選択肢である。

消化器内視鏡による薬剤耐性腸内細菌科細菌のアウトブレイクがしばしば報告されており、主には消化器内視鏡の消毒が不十分であることが原因とされているが、消化器内視鏡が適切に消毒されていたとしても、送水ボトル中の水が汚染されていると、せっかく消毒した内視鏡が再汚染することになる。わが国で 2013 年に作成された消化器内視鏡の感染制御に関するマルチソサエティ実践ガイドでは、送水ボトルは「使用後に洗浄と乾燥を毎日行い、少なくとも週 1 回滅菌する」となっている。本研究はこの推奨を支持するものである。皆さんの施設では送水ボトルは適切に管理されているだろうか。
