背景

多剤耐性スタフィロコッカス・カピティス(Staphylococcus capitis)クローンである NRCS-A  が、イングランドや世界各国で、新生児集中治療室(NICU)の低出生体重児における遅発性敗血症に関連することが多くなっている。この細菌が NICU 環境内のどこで生存・存続するのかを理解することは、効果的な制御策を策定し、実行するための鍵となる。

目的

S. capitis が NICU 内の表面に定着する可能性を検討した。

方法

検体採取時点で NRCS-A 保菌/感染例が存在したまたは存在しなかった 4 施設の NICUから、表面スワブを収集した。検体を培養し、S. capitis 分離株を全ゲノムシークエンシングにより分析した。プラスチック表面での NRCS-A の生存を経時的に評価し、非 NRCS-A 分離株の生存と比較した。一般的に用いられる化学消毒剤の S. capitis に対する殺菌活性を評価した。

結果

検体を採取した表面 173 カ所中 40 カ所(21.1%)に S. capitis が存在し、分離株 30株(75%)が NRCS-A であった。S. capitis は NICU 全体の表面から回収されたが、NRCS-A クローンは、直近の新生児ベッドスペース以外から回収されることはほとんどなかった。保育器やその他のベッドサイド装備は、臨床症例の検出の有無にかかわらず、NRCS-A に汚染されていた。清掃がない場合、S. capitis は 3 日間生存可能であり、生存能の低下は最小限であった(< 0.5 log10 の低下)。トロクロセンナトリウムおよび QAC(四級アンモニウム化合物)ベースの洗浄/消毒剤により、S. capitis は検出可能レベル未満に減少した。

結論

S. capitis NRCS-A は、S. capitis 感染の臨床症例が最近報告されていない病棟であっても、NICU 環境から容易に回収されることから、新生児医療環境内の清掃に関する適切な国のガイダンスの必要性が強調される。
