Duodenoscopy: an amplifier of cross-transmission during a carbapenemase-producing Enterobacteriaceae outbreak in a gastroenterology pathway

2015 年 10 月に同じ十二指腸内視鏡による検査を受けた患者 5 例において、カルバペネマーゼ産生腸内細菌科細菌として OXA-48 産生肺炎桿菌（Klebsiella pneumoniae）が同定された。この内視鏡は、これらの症例間における唯一の疫学的なリンクであった。不十分な消毒プロセス後の十二指腸鏡の一過性コンタミネーションが伝播の原因であった可能性がある。

海外における十二指腸内視鏡を介したカルバペネム耐性腸内細菌科細菌のアウトブレイクの報告に対応して厚生労働省医政局地域医療計画課が「十二指腸内視鏡による多剤耐性菌の伝播について」と題して情報提供を行ったのは 2015 年 3 月のことである。十二指腸内視鏡には鉗子台や鉗子起上ワイヤーチャンネルといった特殊な構造があり、その部分が十分に洗浄・消毒できないことが原因と考えられている。本論文はこれに関連した一連の論文の 1 つと言うことができよう（しかし結局この十二指腸内視鏡からは CPE は検出されていないのだが）。
