Active surveillance for carbapenem-resistant Enterobacteriaceae, vancomycin-resistant enterococci and toxigenic Clostridium difficile among patients transferred from long-term care facilities in Korea

長期ケア施設から病院に転院した患者を対象に 10 か月間のアクティブサーベイランスを実施し、カルバペネマーゼ産生腸内細菌科細菌（CPE）、バンコマイシン耐性腸球菌（VRE）および毒素産生性クロストリジウム・ディフィシル（Clostridium difficile）の保菌について評価した。直腸検体 282 個から、カルバペネム耐性腸内細菌科細菌（CPEは認められなかった）を有する患者 4 例（1.4％）、VRE を有する患者 59 例（21％）、毒素産生性 C. difficile を有する患者 20 例（7.1％）が判明した。本研究期間中に VRE 感染症のアウトブレイクは発生しなかった。CPE 保菌の検出率が低かったことから、長期ケア施設からのすべての入院について CPE のスクリーニングを行うことは費用効果的でないこと、また VRE を対象としたスクリーニングおよび接触予防策の実施については再考すべきであることが示唆された。

監訳者コメント：
長期療養型施設における多剤耐性菌の保菌については、本邦でも報告が増えつつある。MRSA や ESBL 産生グラム陰性桿菌が対象となりやすく、ESBL 産生菌の保菌割合が高いことも指摘されているが、本検討のように CRE、C. difficile まで調査対象を拡げるかについては現在不明確である。各国でこのような取り組みが進むであろうが、知見の蓄積と解析、ならびに対策立案とその効果予測は今後もますます重要になるであろう。
