背景
不適切な人員配置と作業量は、集中治療室(ICU)におけるメチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)の伝播に影響を及ぼす可能性がある。MRSA の新規獲得について調査することにより、ICU における MRSA 獲得と人員配置との関連が明確になると考えられる。
目的
人員配置および病床利用率は、十分な人員を擁する ICU において MRSA 新規獲得件数に即時性および遅延性の影響を及ぼすか、およびこれらの因子を MRSA 獲得の予測因子として使用することが可能かどうかを明らかにすること。
方法
オーストラリアの都市部の 796 床の病院の ICU における 2003 年 1 月から 2006 年 12 月の MRSA 新規獲得に関するデータを用いて、2007 年に実際に観察された MRSA 新規獲得の確率を予測するモデルを作成した。受信者動作特性(ROC)分析によりクロス検証を行った。
結果
対象期間中の 51 週で、61 件の MRSA 新規獲得(感染 21 件、保菌 40 件)が特定された。非正規雇用職員の労働時間は相対的に少なかった。クロス検証分析による曲線下面積(AUC)は 0.46(95%信頼区間 0.25 ~ 0.67)であり、このことから、2003 年 から 2006 年のデータに基づいて作成したモデルによって、2007 年に MRSA 新規獲得が発生する週を予測できないことが示された。
結論
ICU での作業量が多いことがもたらすリスクは、感染制御策の良好な遵守、看護師の訓練、および適切な人員配置比によって低下した可能性がある。したがって、当 ICU の人員配置方針と感染制御策を、MRSA 新規獲得率を制御するために変更する必要はない。

監訳者コメント
この論文は、オーストラリアの看護師:患者が 1:1 で配置されている ICU における MRSA 新規獲得件数とベッド稼働率について検討している。これまでの知見であるスタッフの不足や過重労働が手指衛生や感染制御策の遵守を低下させることを否定するものではなく、むしろ、スタッフの配置基準の設定が感染管理において重要な因子であることを裏付けた。
