Specialized cleaning associated with antimicrobial coatings for reduction of hospital-acquired infection: opinion of the COST Action Network AMiCI (CA15114)

医療従事者の手指衛生遵守の不良に関して認識されている問題や、化学的に消毒済みの表面への手指接触による再汚染に関する報告は、現在利用可能な衛生法に加えて新たな衛生法の必要性を強調している。このようなアプローチの 1 つとして抗菌（ナノ）コーティングがあり、それによって、総合的な有効成分が、処理表面と接触する微生物の減少に寄与する。このことは実験的条件下では広く研究され、有望な結果が得られているが、実際の医療条件下での研究は少ない。医療関連感染の低減を図る抗菌（ナノ）コーティングに伴う特殊洗浄に関して、欧州 30 か国の参加者 75 名の見解を照合した。
従来または新たなプロセスを用いた抗菌（ナノ）コーティングの洗浄のための科学的ガイドラインの策定が必要であるという全員一致の合意がみられた。具体的なトピックは、洗浄剤の作用機序ならびに従来のコーティングや抗菌（ナノ）コーティングと洗浄剤との物理的相互作用を理解すること、評価では反復洗浄やその他劣化の側面（日光の曝露など）の影響を判定するために、コーティングのライフサイクルを再現すること、廃水中の抗菌（ナノ）コーティング由来の殺生物剤の濃度を測定すること、洗浄廃水の有効な無害化ならびに滅菌処理法を開発することであった。さらに、統一見解として、抗菌（ナノ）コーティングの広範な実施に先立ち、関与する臨床・医療管理、洗浄サービス、および環境安全に関する利害関係者のさまざまな責任を明確にする必要があるということが示された。

監訳者コメント：
医療環境の抗菌コーティング技術については、業界内で標準化された物差しがないことが利用する側からすると問題である。日本には EPA もなく、欧州よりはるかに遅れた体制しかなく課題を残している。
