Assessment of theatre shoe contamination in an orthopaedic theatre

人工関節感染症は関節形成術の深刻な合併症のひとつである。多数のプロトコールにより手術中の人工関節感染症の潜在的なリスクは軽減されるが、手術室用靴の管理に関するプロトコールは存在しない。我々の目的は、手術室用靴上の、人工関節感染症を引き起こすことが知られている細菌を評価することであった。40 の手術室用靴が分析され、コアグラーゼ陰性ブドウ球菌（coagulase-negative staphylococci）は 65％（N = 25）に、メチシリン感受性黄色ブドウ球菌（meticillin-susceptible Staphylococcus aureus）は 40％（N = 16）に、メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus）は 25％（N = 10）に存在した。飛び散った血の量は微生物汚染にほとんど相関しなかった。抗菌薬耐性株を含め、靴に寄生しているグラム陽性細菌は、手術室環境への潜在的な伝播経路をもたらしている。

監訳者コメント：
整形外科手術の際の医療従事者の「靴」の汚染に関する論文である。思った以上に高率（25％！）に靴が MRSA で汚染されている。ただこの靴は、複数の医療従事者で共有されていたり、洗濯される頻度もバラバラのようなので…日本での状況とはかなり違いそうである。ただいずれにせよ、「床は医療関連感染症に関連が低い」ことが定説であったが、近年では床に注目が集まりつつある。今後もこの領域の研究は増加しそうである。
