Prevalence of multidrug-resistant organisms in migrant children admitted to an Italian cardiac surgery department, 2015-2016

多剤耐性病原体（MDRO）の保菌を検出するための入院時のスクリーニングは、頻繁に議論されるトピックである。2015 年から 2016 年の間に、心臓外科手術の候補者である 141 例のイタリア人の子供および、354 例のイタリア国外より移住した小児において実施した、微生物学的スクリーニングの結果を報告する。全体で、イタリア人の子供の 25％および、アフリカ人とルーマニア人の子供の 65.4％以上が、1 種類以上の MDRO（メチシリン耐性黄色ブドウ球菌［meticillin-resistant Staphylococcus aureus］、基質特異性拡張型β-ラクタマーゼ産生菌[extended-spectrum β-lactamase enzymes]、カルバペネマーゼ産生菌［carbapenemase producers］、バンコマイシン耐性腸球菌［vancomycin-resistant enterococci］）を保菌していた。本結果に基づき、感染制御の向上のために、地理的に限定しないアプローチが必要であることを提案する。

監訳者コメント：
MDRO の感染と伝播には、国外からの持ち込みによるものが相当数関与していると推測できるが、それはヒト・モノの行き来がより頻繁な国々の状況を反映しやすいであろう。本検討が行われたイタリアの医療機関には、チュニジア、ルーマニア、エジプトからの小児が手術目的で多く紹介されるが、それらは患者特性のみならず MDRO それぞれの分離状況にも差があった。さらに地中海周辺国を中心として様々な国々から患者の受診・搬送があり、分離状況はより複雑になっていた。このような状況での感染制御上の対策の立案と実行は、個々の出身国やそれぞれの医療機関を眼中に置くよりも、国家、地域、保健衛生のレベルでなされなければならないことを示す好例といえよう。
