高度な感染制御対策が蘇生後の臨床転帰に及ぼす影響★

アウトブレイク発生中の救急蘇生に対する感染制御対策の影響は明らかではない。今回の後向き観察研究の目的は、感染制御対策実施後の救急蘇生の転帰を検討することである。2003年1月1日から7月4日までの期間に、台湾中部の1,732床の三次病院でデータを収集した。救急蘇生を必要とする非外傷患者を、厳格な感染制御対策の実施前(期間1)および実施後(期間2)の2群に分けた。解析対象とした変数は、患者の人口統計学的データ、蘇生を行った場所、蘇生に参加した人数、応答時間と蘇生時間、発熱、肺炎の状態、および蘇生の結果であった。期間1と期間2で応答時間に変化はなかったが、緊急挿管せずに患者蘇生を行った数、急速挿管を行った数、「蘇生をしない(do not resuscitation;DNR)」指示数は増加し、期間1ではそれぞれ88件(24.4%)、23件(6.4%)、16件(4.4%)であったのに対し、期間2では103件(33.0%)、32件(10.3%)、29件(9.3%)であった。蘇生失敗率は期間2で有意に高かった(オッズ比1.59、95%信頼区間1.17~2.16)。これらの2つの期間で、発熱または肺炎患者で救急蘇生を行った数に有意差は認められなかった。結論として、厳格な感染制御対策の実施は、救急蘇生の失敗率増加に寄与すると考えられた。感染性が高い疾患のアウトブレイクの次回発生時の救急蘇生処置中には、患者に標準的な医療を提供すること、および十分な医療従事者の適切な保護を図ることが最も重要と思われる。
