Compliance with clothing regulations and traffic flow in the operating room: a multi-centre study of staff discipline during surgical procedures

本多施設共同研究では、手術手技時における手術室スタッフの服装規則の遵守とスタッフの動きの評価を行った。監査を行った 1,615 名の服装中、56％が 服装の 8 つの指標を遵守していた。不遵守の主要な原因は、不適切な宝飾品の装着（26％）およびヘッドカバー（25％）であった。観察した 212 件の手技中、中央値で 5 名（四分位範囲［IQR］4 ～ 6）が切開時に立ち会っていた。手術室の入退室の頻度は中央値で 10.6 回／時（IQR 6 ～ 29）（範囲0 ～ 93）であった。手術室の入退室の主要な理由は、手術室で必要な物品を得るためであった（N ＝ 364、44.5％）。服装規則の遵守率が低い手術室はスタッフの動きが多い傾向があったが、有意差ではなかった（P ＝ 0.12）。

監訳者コメント：
手術後創感染は手術室環境や手術室スタッフに起因する可能性があり、微生物による室内空気の汚染と人工関節手術後の創部汚染との関連が報告されている。したがって手術室内での微生物の浮遊を極力抑制するためにはスタッフの服装や移動制限が必要である。しかしながら、現状では手術室の出入りは頻回であり、手術室内の換気による気流は乱され換気効率を下げるため、創部への汚染リスクを高めることとなる。手術室スタッフの服装と行動についての規制強化が必要である。
