適切な経験的抗菌薬療法により集中治療室で獲得された腸内細菌科細菌菌血症に関連する死亡および退院が変化するか?

背景
集中治療室(ICU)で獲得された腸内細菌科細菌菌血症の転帰、ならびに適切な経験的抗菌薬療法による潜在的な修飾効果については、矛盾する結果となっている。
目的
上記の関連性について、因果関係を推定した文献に記載された方法を用いて潜在的な時間依存性交絡因子を補正して評価すること。
方法
本コホート研究に、2002 年から 2006 年にイングランドの 2 つの一般 ICU に 3 日以上滞在した患者を組み入れた。逆確率重み付け法による周辺構造モデルを用いて、腸内細菌科細菌菌血症に関連した死亡率および退院率、ならびに適切な経験的抗菌薬療法がこれらの転帰に及ぼした影響を推定した。
結果
3,411 件の ICU 入室のうち、195 件(5.7%)でICUで獲得された腸内細菌科細菌菌血症症例が発生した。腸内細菌科細菌菌血症は、ICUにおける死亡の 1 日あたりリスクの上昇(原因特異的ハザード比[HR]1.48、95%信頼区間[CI]1.10 ~ 1.99)ならびに ICU 退院の 1 日あたりリスクの低下(HR 0.66、95%CI 0.54 ~ 0.80)と関連した。適切な経験的抗菌薬療法により、ICU における死亡(HR 1.08、95%CI 0.59 ~ 1.97)または退院(HR 0.91、95%CI 0.63 ~ 1.32)に有意な変化はなかった。
結論
ICU で獲得された腸内細菌科細菌菌血症は、ICU における死亡の 1 日あたりリスクの上昇と関連した。さらに、適切な方法を用いて時間依存性交絡因子を補正した場合も、1 日あたりの退院率も感染症獲得後に低下した。適切な経験的抗菌薬療法により ICU の死亡および退院に対する有益な修飾効果があるというエビデンスは認められなかった。

監訳者コメント
ICU では重篤な患者が多く、必ずしも感染症だけが重症化や死亡要因ではないことから、人工呼吸器関連肺炎の長期予後などの疫学調査においても改善効果を検証する際に有意差が出にくいことが知られている。
