背景
手術室において、開扉率や在室者数と微生物空気汚染との関連を調査した現行の文献は限られている。手術部位感染のリスクが高い低中所得国での研究が特に必要とされる。
目的
ガーナの教育病院の手術室における微生物空気汚染、および開扉・在室者数と微生物空気汚染との関連を評価すること。さらに、開扉の理由を記録することを目的とした。
方法
124 件の待機的な清潔または準清潔手術中に、MAS 100® portable impactor を用いて自動エアーサンプリングを実施した。試験済みの構造化観察フォームを用いた直接観察により、在室者数、開扉率、開扉ごとの理由を記録した。
結果
手術中に、コロニー形成単位(cfu)数の平均は 328 cfu/m3(空気)で、510 サンプル中 429 サンプル(84%)は推奨レベル 180 cfu/m3 を超えていた。記録された開扉 6,717 回のうち 77%は不必要とみなされた。cfu/m3 のレベルは在室者数(P = 0.001)、開扉数/h(P = 0.02)と強く相関した。無人の手術室では、1 時間の連続換気後の平均 cfu 数は 39 cfu/m3 で、102 サンプル中 52 サンプル(51%)は推奨レベル 35 cfu/m3 を超えていた。
結論
本研究では、手術中の空気中の cfu が推奨レベルを超える高値を示した。低中所得国の施設において、微生物空気汚染を低減するために、手術中の開扉回数および在室者数を最小限に抑えることが有効な対策となりうる。

監訳者コメント
ガーナの教育病院の手術室における空気汚染を調べた論文である。微生物の定量、スタッフの出入りを調べたところ相関があったという結果は想像がつくが、77%が不必要な出入りであったという結果に驚いた。まず、対応すべき点であると思われた。
