Infection control implications of the laundering of ambulance staff uniforms and reusable mops

救急医療における洗浄に関して発表されている研究は少ない。我々は、汚れた制服と汚れていない制服、ならびに大腸菌（Escherichia coli）、黄色ブドウ球菌（Staphylococcus aureus）およびクロストリジウム・ディフィシル（Clostridium difficile）芽胞で人為的にコンタミネートした再使用可能なモップヘッドについて細菌培養を行った。救急部門で現在ルーチンに行われている洗浄手順は、本研究のシミュレーションから、病原性栄養細菌を、3.398 log₁₀ コロニー形成単位未満（黄色ブドウ球菌および大腸菌）という検出不能レベルにまで低減する効果を有するようである。洗浄後に C. difficile のレベルは減少したものの検出可能であったが、環境内における C. difficile 芽胞の背景レベルが不明であるため、これが感染に及ぼすリスクは不明である。

監訳者コメント：
医療従事者の衣服の汚染と医療関連感染症との関係は気になるところである。例えば白衣はもちろん、ネクタイや靴などが MRSA で汚染されているが医療関連感染症と関連するという報告がある⁽¹⁾。本研究はそこから一歩進んで、衣服や清掃用具の洗濯の質を評価したもので、特に洗浄後でも C. difficile が残っているのは興味深い。以前日本でタオルのバシラス属による汚染が報告されているが ⁽²⁾ 、芽胞形成菌の汚染は思った以上にしつこいのかもしれない。
