自由生活アメーバはいたるところに存在する原虫であり、生存のために宿主を必要としない。水中や土壌などの自然環境内や、水道水や水泳プールなどの人工環境内に存在しており、そこでは細菌、真菌、およびウイルスなどの他の微生物と相互作用をしていると考えられている。自由生活アメーバは病院の給水システム中に存在する微生物などを内部に取り込むことができるため、そうした微生物を不適切な環境から保護したり、微生物の伝播を媒介したり、マクロファージに貪食された後でも微生物の生存が可能な場を提供したりする。レジオネラ・ニューモフィラ(Legionella pneumophila)と自由生活アメーバとの相互作用については詳細な研究が行われている。その後の研究から、自由生活アメーバはその他の抗酸菌、緑膿菌(Pseudomonas aeruginosa)、アシネトバクター・バウマニー(Acinetobacter baumannii)などの細菌や、真菌、ウイルスのリザーバにもなることが判明している。病院の給水システム内に存在するアメーバは、病原微生物のリザーバとして働くことがあるため、医療関連感染と間接的に関係しているといえる。

施設内における水系感染は、医療関連感染の中で、その存在と実態の把握が非常に難しいものである。このため十分に理解されず、重要性が認識されにくかったといえる。このレビューは医療環境における自由生活アメーバの生態と医療関連感染への関与、ならびにその内部に包含した病原微生物との相互作用、リザーバとしての伝播について、短いながらも多くの情報を含んでいる。施設内の水系感染について、このような視点から各施設での検討が進むことを期待したい。
