Is faecal microbiota transplantation an option to eradicate highly drug-resistant enteric bacteria carriage?

カルバペネム耐性腸内細菌科細菌（CRE）またはバンコマイシン耐性腸球菌（VRE）保菌は、公衆衛生上の重大な課題となっている。除菌のストラテジーは不十分である。本研究は、CRE または VRE の消化管保菌が認められた患者のコホートで、糞便細菌叢移植の影響を評価することを目的とした。8 例の患者を研究に含めた。内訳は、6 例が CRE、2 例が VRE 保菌者であった。糞便細菌叢移植の 1 か月後に、2 例で CRE 保菌が認められず、3 か月後には別の 1 例で VRE が検出されなかった。我々の経験では、このストラテジーは安全である。

監訳者コメント：
糞便細菌叢移植（FMT）後に薬剤耐性菌が便から検出されなくなった例や、便のメタゲノム解析で FMT 前に認めていた薬剤耐性遺伝子が FMT 後には消失した例が報告され、腸内細菌叢を変える FMT が耐性菌の保菌を抑制する可能性が指摘されている。本検討は少数例ながら CRE、VRE 保菌者での FMT の効果をみたものであるが、3 か月で約 4 割で除菌が確認された。症例数を増やし、FMT の方法を工夫した今後の検討が待たれるところである。
