Effectiveness of the CareCentre^® at improving contact precautions: randomized simulation and clinical evaluations

背景
ベッドサイド衛生は、医療関連感染率を低下させるために重要である。CareCentre^® は、擦式アルコール製剤、手袋、エプロン、廃棄物入れ、人間工学に基づいた筆記できる台を備えた病院ベッド端のテーブルである。
目的
ベッドサイド衛生の改善における CareCentre^® の効果を明らかにすること。
方法
ランダム化クロスオーバーシミュレーションの評価において、参加者 20 名が通常のベッドサイドでの作業を実施するために、CareCentre と標準条件を用いた。今回のランダム化クロスオーバー臨床評価では、急性期成人患者用病室 9 組でCareCentre と標準条件をそれぞれ 1 週間使用した。研究者らは、世界保健機関（WHO）の「私の手指衛生の 5 つのタイミング」遵守、ベッドサイドでの手袋およびエプロン着用と廃棄を評価した。
結果
手指衛生ガイドラインの遵守は、シミュレーションにおいて 48％から 67％に（P ＝ 0.04）、臨床評価において 14％から 40％に（P ＜ 0.001）改善した。ベッドサイドでの手袋着用と廃棄は、シミュレーションにおいて19％から79％に（P ＜ 0.001）、臨床評価において 30％から 65％に（P ＝ 0.014）改善した。ベッドサイドでのエプロン着用と廃棄は、シミュレーションにおいて 14％から 78％に（P ＜ 0.001）、臨床評価において 10％から 53％に（P ＝ 0.180）改善した。
結論
CareCentre は、ベッドサイド衛生を改善した。また、集学的戦略の一環として医療関連感染率の低下に有用である可能性がある。

筆者らは CareCentre というオールインワンのサイドデスクをデザインし、その効果を調べた。とても興味深い取り組みであるが、論文中に掲載されている写真では、 個人防護装備（PPE）とゴミ箱が同じ高さで、膝あたりの高さに配置されており、汚染されるのでは、と懸念を感じた。
