Silver-impregnated external-ventricular-drain-related cerebrospinal fluid infections: a meta-analysis

背景
脳脊髄液感染は、脳室ドレーン留置に関連する主要な合併症である。銀含浸脳室ドレーンカテーテルの使用は神経外科センターの多くで一般的になっている。
目的
銀含浸脳室ドレーンカテーテル使用が脳脊髄液感染に及ぼす効果を評価すること。
方法
Medline、Embase、Cochrane Libraryを系統的に検索して、メタアナリシスを実施した。銀含浸脳室ドレーンカテーテルと通常の脳室ドレーンカテーテルとを比較したすべてのランダム化対照試験および非ランダム化対照試験を特定し、解析した。
結果
非ランダム化対照試験6件を対象とした。粗感染率は、通常のカテーテルが 10.8%、銀含浸カテーテルが 8.9%(統合オッズ比[OR]0.71、95%信頼区間[CI]0.46 ~ 1.08、P = 0.11)であった。微生物スペクトル解析において、銀含浸カテーテルはグラム陽性菌を原因とする脳脊髄液感染率が有意に低かった(銀含浸カテーテル群と通常のカテーテル群との比較 2.0%対 6.7%、統合 OR 0.27、95%CI 0.11 ~ 0.63、P = 0.002)。
結論
銀含浸脳室ドレーンカテーテルの抗菌効果は選択的である可能性があり、前向き対照研究でさらに評価する必要がある。

監訳者コメント
脳室ドレーンは頭蓋内圧のコントロール目的で挿入されるが、重大な合併症に脳脊髄液の感染症がある。銀含浸脳室ドレーンカテーテル(A-EVD)は、抗菌スペクトルが広いことで知られているが、今回のメタアナリシスでは、全体の感染率減少効果は認められなかった。分離菌の約 80%を占めるグラム陽性菌と主たる起炎菌であるブドウ球菌への感染率減少効果が両者で認められたが、グラム陰性菌での A-EVD による感染率減少効果はなく、感染率は対照群と比較して高率(33%対 17%)であった。A-EVD の有用性に関しては、比較試験の質と量ともに不十分であり、A-EVD の有用性を結論づけるにはさらなる検討が必要である。
