Panton-Valentine leucocidin(PVL)とγ-ヘモリシンは、黄色ブドウ球菌(Staphylococcus aureus)が産生するsynergohymenotropic毒素ファミリーに属しており、それぞれpvl遺伝子とhlg遺伝子にコードされている。PVL毒素と皮膚や粘膜などの壊死病変との関連については多くの報告がある。本研究の目的は、pvlまたはhlg遺伝子を有する黄色ブドウ球菌株の保有率を判定し、pvlまたはhlg陽性黄色ブドウ球菌と特定の臨床症状との関連を調べることである。2005年1月から2007年7月に、黄色ブドウ球菌合計143株(メチシリン耐性黄色ブドウ球菌[MRSA]58株、メチシリン感性黄色ブドウ球菌85株)のpvlおよびhlg遺伝子を、多重PCR法によりスクリーニングした。これらの株が分離された患者141例の人口統計学的データと臨床データを記録した。pvl遺伝子陽性は31株(21.7%)、hlg遺伝子陽性は77株(53.7%)であった。pvl遺伝子陽性株のうち21株(67.7%)がMRSAであった(P = 0.001)。pvl遺伝子陽性株のうち16株(51.6%)が市中獲得型であった。pvl遺伝子は皮膚・軟組織感染症、特に膿瘍(分離株の60%、P = 0.008)やせつ腫症(分離株の55.5%、P = 0.036)と強く関連していた。これらの結果から、pvl遺伝子陽性株は皮膚感染症および黄色ブドウ球菌のメチシリン耐性と関連することが確認された。

監訳者コメント
同じ病原微生物の菌種でも、菌株の中には毒素を持ち著しい病原性を発揮するものがいて、それをinvasive strain(侵襲株)と呼ぶという考え方がある。この考え方自体は疫学が盛んな欧州を中心に展開されているため、米国や日本などの学術圏では馴染みが少ないかもしれない。我々の学術圏の考え方は、pvl遺伝子をノックダウンさせた株を動物に感染させて、皮膚病変が軽微になることを証明するアプローチを考えるが、倫理上の問題があって実施が困難になるところである。このような違った角度からのアプローチは、米国だけの価値観で作られた学術圏でだけ勉強していてはなかなか発想しにくいところである。Look EU!
