背景
スウェーデンの病院では、清掃請負業者を決定する際に求められる清掃仕様書の中で、視認評価を補うための質管理方法としてアデノシン三リン酸(ATP)法が広く採用されている。
目的
患者に近接する接触頻度が高い表面上の ATP 法で測定した生物学的負荷量が介入後に減少したかどうかを調査すること、同一表面上の視認評価と ATP 値との相関を評価すること、および病院の清浄度評価ツールとしての ATP 法の性能を明らかにすること。
方法
中部スウェーデンの地域病院の内科病棟および集中治療室(ICU)で、2012 年から 2013 年にかけて 3 期からなる前向き介入研究を実施した。従来の清掃手順を明確化し、病室の接触頻度が高い表面 10 か所を対象として、介入前後に視認検査および ATP 測定を行った。介入の内容は、看護職員に対する病院清掃の重要性に関する教育、および清掃前後の ATP 値の直接的なフィードバックとした。
結果
混合モデルにより、ATP 値は介入後に有意に減少したことが示された(P < 0.001)。相対発光量(RLU)は ICU のほうが低かった。視認評価により判定した清浄度は改善した。ロジスティック回帰分析により、視認評価と ATP 値との間に有意な関連が認められた。
結論
ATP 値の直接的なフィードバックと、教育および文書による清掃プロトコール導入の併用は、清浄度改善のための有効なツールであった。視認評価と ATP 値との間に関連がみられたが、相関は強固ではなかった。ATP 法は職員の教育ツールとなり得るが、広いエリアの限られた部分を対象としたものであるため、病院全体の清浄度を評価するためには十分なものではない。

監訳者コメント
スウェーデンの 76 床の単施設での研究である。介入開始前は、病室の掃除に関するマニュアルはなく、特に教育も受けていなかった。フィードバックとして視認法と ATP 法の評価をしているが、いずれにしても、マニュアルを作成し、スタッフに教育を行ったことが清浄度改善に最も寄与したのではないだろうか。
