1940 年代に導入された予防的抗菌薬投与は、大腸手術が比較的安全である時代をもたらした。その一部は、腸内細菌科細菌に起因する手術部位感染症（SSI）の予防によって達成されたものである。それ以来、予防投与に高頻度で用いられる抗菌薬に対する腸内細菌科細菌の耐性が、徐々に増大してきた。術前の耐性腸内細菌科細菌保菌が大腸 SSI 予防投与の効果に及ぼす影響は、もしあるとしても明らかではない。また、手術時の予防的抗菌薬投与の用量反応関係が明確にされていないため、耐性の影響を予測することは困難である。さらに、どのような耐性検査法を用いるべきかも明らかではない。結腸内の腸内細菌科細菌濃度の重要性、腸内細菌科細菌の種々の菌種が SSI を引き起こす能力、および最小発育阻止濃度や耐性機序による SSI リスクの相対的な予測能については、まだ確立していない。これらの疑問に答えるためには臨床研究が緊要である。
