Catheter-related infection in Irish intensive care units diagnosed with HELICS criteria: a multi-centre surveillance study

背景
医療の質の指標として、カテーテル関連感染症(CRI)サーベイランスのデータを利用することが提唱されている。しかし、アイルランドの集中治療室(ICU)を対象とした全国的 CRI サーベイランスプログラムや標準化された CRI の定義はない。
目的
Hospitals in Europe Link for Infection Control through Surveillance(HELICS)の CRI の定義(CRI 1、CRI 2、および CRI 3)を使用する、アイルランドの 9 病院の ICU を対象とした多施設 CRI サーベイランスの実施可能性について調査すること。
方法
3 か月間の研究期間中に、ICU に 48 時間以上入室した18歳を超える患者に挿入したすべての非トンネル型中心静脈カテーテル(CVC)を対象とした。
結果
調査用紙の返送率は 99.5%であり、実施可能性が確認された。患者 614 例に対する 1,209 件、7,587 CVC 日の CVC 挿入のデータから 17 件の CRI エピソードが認められ、全国の CRI 発生率は 1,000 CVC 日あたり 2.2 件(95%信頼区間[CI] 1.2 ~ 3.3)であった。CRI 1、CRI 2、および CRI 3 の発生率はそれぞれ 1,000 CVC 日あたり 0.13 件(95%CI 0.00 ~ 0.39)、0.79 件(95%CI 0.16 ~ 1.42)、および 1.39 件(95%CI 0.60 ~ 2.17)であった。CRI の有無は、患者 1 例あたりの ICU 入室期間(P < 0.001)、CVC 件数(P < 0.001)、および合計 CVC 日(P < 0.001)と関連していた。CVC を手術室で挿入した場合の CRI 発生率は、ICU で挿入した場合と比較して高かった(発生率比 3.9、95%CI 1.3 ~ 11.5、P = 0.02)。参加施設の報告ではサーベイランスを実施するうえでの困難はわずかであり、データ収集に要する時間は患者 1 例について 1 週間あたり約 1 時間であった。
結論
本研究から、HELICS による CRI の定義を用いた多施設 ICU サーベイランスは実用的かつ実施可能であり、臨床的に重要な情報が得られることが示された。ICU での CRI サーベイランスは、多忙な環境ではあるものの、CRI の減少を図るため、また現行の CRI 減少のためのプロセス評価を推進するうえで推奨されるものである。
