Epidemiology of infection and current guidelines for infection prevention in cystic fibrosis patients

肺の嚢胞性線維症（CF）にみられる細菌性病原体のスペクトルはこの 10 年間で拡大している。元々関与が判明している緑膿菌（Pseudomonas aeruginosa）、セパシア菌群（Burkholderia cepacia complex）、および黄色ブドウ球菌（Staphylococcus aureus）に加えて、新たにグラム陰性非発酵菌や非結核性抗酸菌が臨床的に重要になっている。入院施設および外来診察室でのエアサンプリング、および患者の咳のエアロゾル分析によって、CF の病原体が空気伝播する可能性を示すエビデンスが得られつつある。CF 患者における「マイコバクテリウム・マシリエンゼ（Mycobacterium abscessus subsp. massiliense）」のアウトブレイクが 2 事例報告され、非結核性抗酸菌の空気伝播についても疑われるようになった。新しい疫学的エビデンスを踏まえて、国際的な感染制御ガイドラインの文書が改訂された。ガイドラインでは、CF 患者入院施設を新規に計画する際の換気方式の重要性について意見が一致しており、病棟廊下および共用エリアでの空気汚染のリスクを低下させるため、入院病室と外来診察室は陰圧室にするよう考慮すべきと述べられている。さらに入院病室と肺機能検査室の換気回数に注意し、可能な限り最適なものとする必要がある。ただし病院環境で患者にマスク着用を求めるべきかどうかについては合意に至っていない。

監訳者コメント：
CF に関与する病原体の種類は様々であり、中には近年になってその位置づけがようやく明らかになってきたものもある。まだ不明の部分も多いが、微生物学的な情報に基づいて、感染制御に関しても具体的な指針を作成できるようになったといえよう。一方、CF で得られた種々の知見は、他疾患や状況においても非常に有用な情報となる。今後の研究の展開が期待される。
