Controlling the spread of vancomycin-resistant enterococci. Is active screening worthwhile?

バンコマイシン耐性腸球菌（VRE）は、医療関連感染の重要な原因となっている。積極的スクリーニング、すなわちリスク患者から直腸スワブや便サンプルを用いて保菌を検出することは、これまでに認知できていなかった症例を特定できるため、予防に寄与すると考えられている。本総説の目的は、VRE スクリーニングが予防・制御、その費用対効果、および最近の検出検査法に及ぼす影響を明らかにすることである。2000 年以降に公表された英語の研究のレビューを行った。種々のガイドライン類が利用・調査されていた一方で、原著論文・研究が重視されていた。スクリーニング対象とすべきと考えられる患者集団は、重症管理室、血液腫瘍内科、および移植部門への入院患者、長期透析患者、ならびに長期滞在型施設から急性期病院への入院患者であることが判明した。積極的スクリーニングは、一部の研究では VRE 保菌・感染の減少および費用の節減との関連が示されていたが、これらはランダム化研究ではなかった。選択培地により検出感度が上昇し、検出時間が短縮するが、分子的手法の意義をさらに検討する余地がある。結論として、積極的スクリーニングは、おそらくは隔離などの制御策に対する意識を向上させることによって、VRE 予防に寄与する。しかし、さらなる研究によって、スクリーニングを必要とする高リスク集団をより明確にし、臨床的・経済的有益性を定量的に評価し、多様な患者集団における最適な検査方法を特定する必要がある。
