Tuberculosis screening in a dialysis unit: detecting latent tuberculosis infection is only half the problem

慢性腎臓病患者では結核発症のリスクが高くなる。本稿では、肺結核に罹患した血液透析患者 1 例の検出と、その後の経験について報告する。初発症例と同時に透析を受けていた患者 41例中9 例がインターフェロン γ 遊離試験（IGRA）陽性と判定され、潜在性結核感染症として治療を受けた。当初は IGRA 陰性であった患者を 6 か月後に再検査したところ、さらに 3 例の IGRA 陽性患者が特定された。その後、12 か月時に全患者の再検査を行った。新規 IGRA 陽性症例は特定されず、職員および患者に活動性結核発症者はみられなかった。IGRA 陽性患者 12 例のうち、潜在性結核感染症の治療を完遂したのは 5 例のみであった。

慢性腎臓病患者において結核はしばしば経験され、再活性化による高頻度の発症に加え、発症者からの曝露による新規の感染も懸念される。透析患者においては特に、透析室での曝露のリスクが高い。本報告は、排菌していた透析患者 1 名が及ぼす曝露のインパクトを IGRA を用いて明らかにした点が興味深い。
