Thirty-day mortality in UK patients with community-onset and hospital-acquired meticillin-susceptible Staphylococcus aureus bacteraemia

背景
市中発症型と病院獲得型の黄色ブドウ球菌（Staphylococcus aureus）感染症患者の死亡率の相違に関する研究はわずかであり、またそれらの結果は一貫していない。
目的
メチシリン感性黄色ブドウ球菌（meticillin-susceptible S. aureus；MSSA）菌血症の連続症例の 30 日死亡率、および市中発症型感染症と転帰との関連を明らかにすること。
方法
MSSA 菌血症患者の人口統計学的、臨床的、および微生物学的データを、2007 年 8 月から 2011 年 7 月に前向きに収集した。患者の追跡を死亡、退院、または感染症の回復まで行った。多変量ロジスティック回帰を用いて、市中発症型感染症と 30 日死亡率との関連を明らかにした。
結果
患者 392 例に合計 403 件の菌血症エピソードが発生した。全死亡数は 7 日後の時点で 44 例（11.2％、95％信頼区間［CI］ 7.9％ ～ 14.0％）、30 日後の時点で 101 例（25.8％、95％CI 21.5％ ～ 30.4％）であった。30 日死亡率は市中発症型（256 例中 71 例、27.7％）と病院獲得型（147 例中 31 例、21.1％）の感染症患者の間に相違が認められた。感染性心内膜炎（14 例中 13 例、92.9％）、化膿性脊椎炎（13 例中 12 例、92.3％）、および皮膚・軟部組織感染症（71 例中 61 例、85.9％）は市中発症型のほうが発生頻度が高かったが、血管内留置カテーテル関連感染症（82 例中 60 例、73.2％）は病院獲得型が優勢であった。年齢、Pitt スコア、Charlson 併存疾患指数、特定の感染部位（皮膚・軟部組織、下気道、および末梢関節）、および適切な治療の遅延は、30 日死亡率と強く関連していた。年齢、Charlson 併存疾患指数、および適切な治療の遅延で補正した多変量解析では、市中発症型感染症と 30 日死亡率との間に強い関連が認められた（オッズ比 1.59、95％CI 0.91 ～ 2.80）。
結論
病院獲得型 MSSA 菌血症と比較して、市中発症型の感染症は 30 日後の転帰が不良であった。病院獲得型 MSSA 菌血症は異所性の感染症を引き起こすことはまれであり、医療器材との関連がみられることが多く、患者の転帰は良好であった。
