A prospective study of nosocomial-infection-related mortality assessed through mortality reviews in 14 hospitals in Northern France

目的
フランスの病院で発生した院内感染関連死の件数を明らかにすること、および院内感染関連死の予防可能性を評価するうえでの病院の死亡率調査の意義を検討すること。
方法
本研究の対象は、2007 年から 2008 年にフランスの 14 病院で発生した全死亡 13,537 件である。死亡した患者を担当していた医師や看護師らを含む専門家委員会が、死因が院内感染症である可能性と、院内感染および死亡の予防可能性を判定した。
結果
McCabe スコアが 0 または 1 で、入院後 48 時間以降に死亡した適格患者 2,355 例のカルテレビューを行った。これらの患者の 33％が 1 件以上の院内感染症に罹患していた。死因が院内感染症であると考えられる患者は 182 例、このうち死亡が予防可能であったと判定された症例は 35 例であった。このうち 10 件の死亡は予期されないものであった。
結論
これらの結果を全国に当てはめると、フランスでは院内感染症による死亡が年間約 3,500 件（95％信頼区間［CI］ 2,605 ～ 4,036）発生していることになる。このうち、約 1,300 件（95％CI 357 ～ 2,196）の院内感染と 800 件（95％CI 51 ～ 1,481）の死亡は予防可能であったと考えられる。病院の死亡率調査委員会は、死因となる院内感染症をもたらす環境を特定し、具体的な予防対策を策定することによって、医療の質改善に寄与し得る。このような病院の委員会には、あらゆる医療従事者が参加すべきである。
