Varicella zoster virus infection among healthcare workers in Taiwan: seroprevalence and predictive value of history of varicella infection

背景
水痘・帯状疱疹ウイルス（VZV）は、感染した医療従事者から同僚や患者に伝播する可能性がある。自己報告による水痘の既往歴が、VZV に対する免疫を有していることの証明であると見なされることがある。
目的
台湾の 3 次ケア病院の医療従事者を対象として、記憶に基づく VZV 感染歴と VZV 抗体陽性との関連を明らかにすること。
方法
2008 年 5 月から 2009 年 4 月に、台湾の 3 次ケア病院の全医療従事者が VZV IgG 検査を受け、水痘の既往歴またはワクチン接種歴に関する自記式質問票に記入した。抗体陰性の医療従事者にはワクチン接種を行った。
結果
病院の全医療従事者（3,733 名）が本研究に参加した。参加者の平均年齢は 34.6 歳、VZV 抗体陽性率は 91.1％であった。自己報告による水痘の既往歴の感度 82.3％、特異度 48.6％、陽性的中率 96.3％、および陰性的中率 14.4％であった。水痘ワクチン接種歴については、それぞれ 23.4％、69.4％、90.9％、および 6.5％であった。若年の医療従事者、および医療専門職（医師、看護師、およびパラメディカル）では、記憶に基づく水痘の既往歴の感度は高かった。しかし、有意に高い陽性的中率を示したのは、医療専門職の記憶に基づく既往歴のみであった。
結論
記憶に基づく水痘の既往歴およびワクチン接種歴は、防御抗体 VZV IgG の保有を証明するものではなく、記憶に基づく既往歴がないことは、免疫がないことを予測するものではなかった。院内感染を有効に予防するためには、全医療従事者の VZV IgG の保有状況を確認し、水痘に感受性を有する医療従事者に対してはワクチン接種を実施すべきである。
