Investigation of a cluster of cutaneous aspergillosis in a neonatal intensive care unit

2007 年 12 月から 2008年 7 月に、英国サルフォードの新生児集中治療室（NICU）の 3 例の新生児がアスペルギルス・フミガーツス（Aspergillus fumigatus）による原発性皮膚アスペルギルス症（PCA）と診断された。初発 PCA 症例は 2007 年 12 月に多臓器不全を発症して短期間で死亡し、他の 2 症例は抗真菌薬静脈内投与後の経口 posaconazole による治療で救命した。NICU 内およびこれらの新生児を収容していた保育器から、空気、表面、および水のサンプルを採取した。検出された真菌分離株はすべて A. fumigatus であることが、βチューブリン領域のシークエンシングにより確認された。マイクロサテライトマーカーによる菌株タイピングにより、遺伝子的に関連する A. fumigatus 分離株が新生児および保育器の恒湿器から検出され、感染の原因は NICU で使用していた恒湿器・保育器であることが示唆された。Aspergillus 属菌株のタイピングは、臨床環境でのアウトブレイクにおける感染源の特定、およびその後の感染予防のための環境介入法の計画に有用であると考えられる。

監訳者コメント：

加湿器の医療関連感染に対するリスクを明確に示した論文である。NICU で発生したため問題が顕在化したが、他の病棟なら顕在化しなかったかもしれない。いずれにせよ、インフルエンザの季節に病院環境において加湿を行うことに関する警鐘である。
