Hospital antibiotic consumption in Switzerland: comparison of a multicultural country with Europe

スイスの入院患者の抗菌薬使用量を評価し、同国内の言語圏間の相違を明らかにするとともに、欧州の調査結果との比較を実施した。抗菌薬使用量のデータは、国の急性期病院の 54％が参加するセンチネルネットワーク、および民間の医薬品市場調査会社から得た。収集したデータは 1 日規定用量（DDD）に換算した。スイス全体の使用密度（consumption density）は、欧州の調査で報告された使用量中央値と同程度であった。2004 年から 2008 年にかけて、全病院の全身性抗菌薬の総使用量は 100 病床利用日あたり 46.1 DDD から 54.0 DDD に増加し、集中治療室では 100 病床利用日あたり 101.6 DDD から 114.3 DDD に増加した。総使用量の相違が言語圏間に認められ、また抗菌薬クラス間でも相違がみられた。イタリア語圏の病院の使用密度は有意に高く、次いでフランス語圏、ドイツ語圏であった。さらにイタリア語圏の病院ではフルオロキノロン系の使用量が多く、イタリア、ドイツ、フランスの間に報告されている相違と一致していた。スイスの急性期病院の抗菌薬使用量は欧州の中央値と同程度であったが、集中治療室での使用量は比較的少なかった。欧州諸国間でみられる使用レベルの相違パターンの一部は、スイス国内の文化圏間にも認められた。
