Impact of a blood culture collection kit on the quality of blood culture sampling: fear and the law of unintended consequences

血液培養は敗血症が疑われる患者の重要な検査方法であり、確定診断を得るための第一ステップとなる場合があるが、患者に深刻な悪影響を及ぼすこともあり得る。本研究の目的は、ランカシャー教育病院における血液培養のコンタミネーションの問題がどの程度であるかを明らかにするとともに、新規の血液培養採血キットの導入によるコンタミネーション率への影響を評価することである。当院の血液培養採血キット導入以前の血液培養コンタミネーション率は 9.2％であった。キットの導入後、コンタミネーション率は 3.8％に低下し、American Society for Microbiologists が勧告する標準値である 3％以下に近づいた。コンタミネーションの減少とともに、血液培養サンプリング総数が意図せず、しかも持続的に減少し、またグラム陰性菌菌血症の件数にも、望んでいない減少がみられた。このような減少は、導入時の教育・研修における問題を反映している可能性がある。また現在の「根本原因分析（root cause analysis）」の時代において、血液培養コンタミネーションへの責任が後に判明することへの若い医師の不安を反映している可能性もある。本研究から、血液培養キットの使用継続は推奨されるが、その実施に際しては責任の所在を問わないような運用が求められること、および定期的な教育・研修会が必要であることが示される。
