Nosocomial outbreak of hepatitis B virus infection involving two hospitals in the Republic of Ireland

B 型肝炎ウイルス（HBV）の院内伝播経路は、この何年かの間に変遷を遂げている。輸血関連および医療従事者から患者への HBV 伝播予防のための取り組みにより、これらの伝播経路を介した感染は改善されてきた。最近の HBV 感染症院内アウトブレイクの報告からは、標準予防策の不遵守が HBV 伝播の重要な原因であることが示唆されている。本報告では、2005 年 1 月から 2006 年 3 月の間にアイルランド共和国で発生した HBV 感染症の院内アウトブレイクについて述べる。このアウトブレイクが判明したのは、唯一のリスク因子が直近の外科手術である患者に HBV 急性感染症が確認された後である。その後、複数の施設の広範な調査により、この患者は HBV 急性感染症の 5 症例中の 1 例であること、および 15 か月間に 2 病院で 4 件の感染症症例間の伝播が時間的間隔を置いて発生していたことが判明した。各伝播イベントの明確な原因の特定には至らなかったが、標準予防策、安全な注射、および静脈切開手技の遵守が不十分であることが否定されなかった。院内感染症例 2 例との市中での接触による HBV 急性感染症の二次症例も 2 例確認された。プレコア領域変異を有する HBV の感染や、7 例の患者間でウイルスが伝播したことの確認には、系統解析が有用な手段であることが示された。曝露の可能性のある患者 1,028 例に通知を行ったが、その他の HBV 院内感染症症例のエビデンスは認められなかった。これらの結果から、標準予防策を一貫して実施することの重要性が明確になった。
