Role of overcrowding in meticillin-resistant Staphylococcus aureus transmission: Bayesian network analysis for a single public hospital

病院内の多剤耐性菌の伝播は、相互に関連する多数の因子、すなわち保菌率（蔓延の程度）、手指衛生、患者スクリーニングの有効性、保菌者の隔離・コホーティング、病院清掃の質、病床利用率などに影響される。また、ある多剤耐性菌が蔓延すると、隔離病床利用率が上昇し、隔離のための資源が減少することによって、他の菌の伝播に影響を及ぼす可能性がある。例えば、第三世代セファロスポリン系抗菌薬の過剰使用によって基質特異性拡張型 β-ラクタマーゼ産生肺炎桿菌（Klebsiella pneumoniae）が増加することがあり、このことが間接的にメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）の伝播に影響する。この複雑な相互関連システムの検討のために、本研究ではBayesian ネットワークを使用した。公立病院 1 施設における初期 2 年間の解析結果を報告する。結論として、当施設では、高い病床利用率と MRSA 伝播の増加との関連は、動的な複数の閾値・転換点に依存している可能性がある。この関連は、MRSA の蔓延の程度などの他の因子や、高い病床利用率が新規入院患者のためのベッドの準備・清掃、手指衛生、および保菌者の隔離・コホーティングを阻害するかどうかによる影響を受けると考えられる。

本論文の前に掲載されている報告と同様に、複雑な統計学的解析に基づいて MRSA 伝播と様々な感染対策要因との関連を検討している。MRSA 伝播防止対策は本来多面的であり、本論文の結果は極めて妥当であると考える。
