Epidemiology of nosocomial bloodstream infections in Estonia

院内血流感染（BSI）の調査およびBSIサーベイランスの促進を目的として、2004年から2005年にエストニアにおいて多施設の前向き全病院的サーベイランス研究を実施した。2か所の紹介病院と1か所の中央病院の急性期ケア部門の全患者を対象とした。合計549件のBSIが患者507例で発生した（1,000患者日あたり0.6件）。このうち55％は集中治療室で発生し、47％はカテーテル関連感染であった。BSIの症例のうち24％は血液悪性腫瘍患者に生じていた。院内致死率は31％であった。原因菌のうち315件（53％）はグラム陽性好気性菌、232件（39％）はグラム陰性好気性菌、35件（6％）は真菌であった。嫌気性菌の割合は2％であった。頻度の高かった病原菌はコアグラーゼ陰性ブドウ球菌（26％）、腸内細菌科細菌（24％）、腸球菌（13％）、およびシュードモナス属菌（10％）であった。BSIの8％は複数菌によるものであった。また、黄色ブドウ球菌分離株の7％がメチシリン耐性菌であった。シュードモナス属菌分離株の耐性菌の割合は、セフタジジム19％、メロペネム25％、タゾバクタム・ピペラシリン30％、およびイミペネム44％であった。BSI発生率は、他の既報の研究と有意な差は認められなかった。シュードモナス属菌の抗菌薬耐性率が比較的高いことを除いて、エストニアの院内BSIの病原菌の全体的な耐性パターンは北欧諸国と同様であり、中欧および南欧よりも低かった。本研究は、エストニアの病院でサーベイランスを計画し、実施するにあたって有用である。

エストニアのBSIに関する研究。主に臨床検体分離菌に関する検討を加えている。著者自身も書いている通り、既報の論文とさほど差違はなく、同国の現状を明らかにしたという位置づけにとどまるであろう。
