Promoting hand hygiene in healthcare through national/subnational campaigns

世界保健機関（WHO）の First Global Patient Safety Challenge は 2007 年、医療環境での手指衛生の改善を目的とした大規模な共同活動に関するベースライン調査を実施した。2009 年には、現状の評価および成功に寄与する因子についての情報を得るために再調査を行った。WHO の地域事務局と当該領域の専門家が共同活動の特定を行った。2009 年 3 月から 4 月には、構造化質問票を用いてオンライン調査を実施した。2009 年の調査では合計 38 件のキャンペーンまたはプログラムの担当者が質問票に回答した。38 件中 29 件は国または地域レベルの進行中のイニシアチブであり、このうち22 件（75.8％）は 2005 年 10 月の First Global Patient Safety Challenge の発足後に開始したものであった。これらの共同活動の対象は主として総合病院、地域病院、および大学病院であったが、長期ケア施設およびプライマリ・ケア施設を対象とする割合は2007年の調査と比較して増加した。活動の進捗度は、認知度向上の段階から評価を実施中の大規模な公的活動まで様々であった。半数以上のイニシアチブ（29 件中 20 件）は複数の資金提供を受けており、また、政府は主要な資金提供者の1つであった。29 件のプログラム中 25 件（86.2％）は政府主導によるものであった。First Global Patient Safety Challenge の役割は、推進役としてツールや勧告を提供し、活動を主導・援助することであることが明確に示された。個別の障壁をどの程度重視しているかについては、イニシアチブによって大きな相違があった。いずれの地域でも、資金提供者からの融資に関連する障壁（優先項目とされていないこと、支援が不十分であること）および利用可能な医療資源が重大な問題であった。多くの国・地域の医療環境における手指衛生の促進は、明確な目標、戦略、および施策や医療資源の割り当てを介する政府支援に基づいて行われている。活動の継続にはこれらのことが重要であるが、医療資源の乏しい国々を含む各国間の共同活動を主導するためには、さらなる措置が必要である。
