Frequent MRSA nasal colonization among hospitalized children and their parents in Angola and São Tomé and Príncipe

背景
院内獲得メチシリン耐性黄色ブドウ球菌（methicillin resistant Staphylococcus aureus；MRSA）の鼻腔内保菌率は、以前にはアンゴラのルアンダの小児科病院 1 施設で 23％、サントメ・プリンシペの総合病院 1 施設では 18％と推定された。
目的
アンゴラおよびサントメ・プリンシペの病院 2 施設の入院患児およびその親における黄色ブドウ球菌／MRSA の保菌率を評価すること。
方法
2017 年に、2 か国の入院患児 127 例およびその親 129 例を対象に、黄色ブドウ球菌／MRSA 保菌の検査のために鼻腔スワブを採取した。分離株について、mecA および Panton-Valentine leukocidin（PVL）遺伝子の有無を検査し、パルスフィールド・ゲル電気泳動（PFGE）、spa タイピング、複数部位塩基配列タイピング（MLST）、およびSCCmec タイピングによりその特徴を明らかにした。
結果
患児 127 例中 20 例（15.7％）およびその親 129 例中 13 例（10.1％）に、 MRSA 鼻腔内保菌が認められた。以下の 3 系統が MRSA 分離株の 88％を占めた：（i）PFGE A-ST5-SCCmec IVa（15 例、45％）、spa 型 t105 と関連し、アンゴラのみで回収、（ii）PFGE N-ST8-IV/V（7 例、21％）、spa 型 t008/t121 と関連し、サントメ・プリンシペのみで回収、（iii）PFGE B-ST88-IVa（7 例、21％）、spa 型 t325／t786 と関連し、両国で回収。患児／保護者（親）15 組には、同一の MRSA 株（8 例）またはメチシリン感受性黄色ブドウ球菌株（7 例）の保菌が認められた。PVL は分離株の 25％で検出され、これには MRSA 2 株（ST30-V および ST8-IVa）が含まれた。.
結論
入院患児とその親は MRSA の重要なリザーバである。感染制御策では、市中への MRSA の拡散を最小限に抑えるために、入院患児の親に焦点を当てるべきである。

監訳者コメント：
MRSA 保菌患児の親も一定の確率で同じタイプの MRSA を保菌していることが示された。小児入院患者の管理においては親の協力は必須であるが、感染管理の観点からも手指衛生などを始め、親に協力してもらうことが重要と考えられる。
