Should healthcare workers be screened routinely for meticillin-resistant Staphylococcus aureus? A review of the evidence

英国国民保健サービス（NHS）はメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）が局地的流行の状態にあると認識しており、最近、スコットランドとイングランドでは入院患者に対する日常的な MRSA スクリーニングが導入された。英国国立スクリーニング委員会（National Screening Committee）は、「推奨されるスクリーニングプログラムの適格基準の強化を求める世論を予期しておくべきであり、またこれらに関連するあらゆる決定事項は科学的に正当なものでなければならない」と述べている。文献のレビューを実施し、スコットランドの MRSA スクリーニングの対象を医療従事者の日常的なスクリーニングまで拡張すべきかどうかを評価した。NHS 病院の医療従事者全体の MRSA 保菌率を報告した有病率調査は発表されていない。世界各国の文献に報告されている医療従事者の推定保菌率は、国、病院の専門科、および流行の状態（局地的流行、非局地的流行、またはアウトブレイク）によって大きく異なる。MRSA の局地的流行がみられる病院で実施された最近の研究によると、非アウトブレイク状況下の保菌率は 0％から 15％であった。MRSA の伝播における医療従事者の保菌の役割は十分に明らかにされていない。しかし、持続的な保菌は感染のリザーバとなる可能性があり、アウトブレイクに関する多くの報告では医療従事者が感染源となることが示唆されている。MRSA の局地的流行がみられる病院環境において、MRSA 感染予防・制御のための介入として医療従事者を対象とした日常的なスクリーニングを実施した場合の効果を評価した比較対照試験は報告されていない。医療従事者のスクリーニングに関するエビデンスの多くはアウトブレイクの報告から得られたものであり、これらの報告では、一連の感染制御対策の一環として職員に対するスクリーニングを導入した後、アウトブレイクが終息している。スコットランドの NHS 病院の医療従事者に対する日常的な MRSA スクリーニングの導入を推奨するためには、さらなる研究が必要である。
