Hospital preparedness and clinical description of the 2009 influenza A (H1N1) pandemic in a Belgian tertiary hospital

ベルギーの 3 次病院で成人患者に発生したパンデミックインフルエンザ A（H1N1）2009 の、コストを含めた対策と臨床的特性を報告する。対策委員会がトリアージや入院などの全体的な運営の調整を実施した。2009 年 6 月 1 日から 11 月 30 日に、インフルエンザ様疾患患者 521 例が救急病棟に入院した。A 型インフルエンザと診断された 43 例の入院患者のデータを評価した。年齢中央値は 44 歳（範囲 21 ～ 79 歳）であり、84％が基礎疾患を有していた。11 例が集中治療室への入室を必要とし、1 例が死亡した。インフルエンザの流行による経済的影響は 75,691 ユーロと推計され、このコストの約半分は感染制御対策実践の強化に関連するものであった。今回の入院患者 43 例のコホートでみられたベルギーのパンデミックインフルエンザ A（H1N1）2009 での集中治療室入室率は 26％と比較的高く、死亡率は 3％とごく標準的であった。この後向き研究は将来の流行の制御を改善する上で有用であると考えられる。

2009 年 A（H1N1）については様々な論文により知見が発表されている。本論文は単一医療施設における 43 例のデータのまとめであり、その結果自体に特筆すべきものはみられない。しかし、単一医療施設のデータのまとめ方を知る上で参考になる論文である。また、インフルエンザ流行に際して病院に生じたコストの分析方法も参考にはなる。
