Are national targets the right way to improve infection control practice?

「感染制御の実施を改善する正しい方法」は費用対効果に優れるとともに、感染制御の財源を公平に分配するものでなければならない。費用対効果は「利益」の総体を評価する指標であり、公平性においては同様の状況にある患者に対して同様の治療を実施することが重視される。英国ではメチシリン耐性黄色ブドウ球菌（Staphylococcus aureus）（MRSA）血流感染（bloodstream infection； BSI）がその標的とされているため、英国国民保健サービス・トラストはMRSA BSI予防を目的とする戦略を優先させている。財源が限られた状況下では、MRSA BSIを標的とすることによって、標的以外の医療関連感染のリスクを有する患者は必然的に優先順位が下がるが、これらの医療関連感染の中にはMRSA BSIと同等かそれ以上の不良な転帰に関連するものもある。Health Foundationが提唱するPDSA（Plan, Do, Study, Act）サイクルのような確立された医療改善戦略では、目標（または標的）の設定が必要である。感染制御の実践を改善するためには、何を指標としてそれをどのように評価するか、何を改善する（標的とする）べきかを決定する必要がある。感染予防の標的を選択する際には、不良な転帰全体への医療関連感染の寄与を考慮すべきである。人的なリスク補償行動と微生物の適応はいずれも、全体の転帰とは無関係に感染の標的にかかわる効果全般を妨げる可能性がある。リスクを取るということは健全な医療システムの一環である。医療関連感染の転帰を全体の転帰から遊離させないように、あるいは「リスクを負う者」を「リスク管理者」から切り離さないように注意を払わなければならない。人的なリスク補償行動の余地を制限するとともに、微生物の適応を警戒する必要がある。標的は、公平性と費用対効果を勘案しつつ、地域レベルで設定すべきである。地域に適した情報が重要であり、明確な動機付けがあると非常に効果的である。
